Environmental Protection Specialist Colorado Department of Natural Resources Division of Reclamation, Mining and Safety Office of Mined Land Reclamation 1313 Sherman Street, Room 215 Denver, Colorado 80203 / � v Re: Permit No. M-1980-244: Cripple Creek & Victor Gold Mining Company ("CCWT Cresson Proiect-Written notification — High pH Thickener Tank Spill — 21st May 2017 Dear Mr. Cazier, In accordance with Rule 8, Section 8.2.3 of the Hard Rock, Metal and Designated Mining Operations Regulations (2015), CC&V hereby provides the Division with a formal report regarding the spill of High pH thickener slurry to the lined High Grade Mill area on May 21 st 2017. The following points address the requirements as outlined within Rule 8, Sub section 8.2. a. Actions taken to respond to and correct the emergency situation or condition • Once the release was identified an investigation was immediately commenced. An inspection of the containment area outside the concrete sump was completed and any contaminated soil was collected and returned to the process. • Samples were taken of the previously contaminated area for further analysis. • A skidsteer recovered the contaminated material using an `over-excavation' approach. Depth of recovery was approximately 24 inches and extending about 3 feet around the boundary of the material. • The excavated soil was transported by skidsteer to the Mill crushing circuit. The skidsteer was then decontaminated and returned to normal activities • The area was inspected for any sign of slurry and samples were taken of the soil for verification of full recovery. b. Any known or adverse impacts to human health, property or the environment
