The italicized items are the current comment and the bold text are the <br />responses: <br />APPL ICA TION FORM., Lines 1.1 and 5.2 were left blank, and the 'Applicandoperator or company name" <br />given on line 1 does not match that used on the signature page (page 8). <br />Line 1.1 and 5.2 have been updated and included in the new permit submittal via email and hard <br />copy. <br />EXHIBITS.- <br />Exhibits <br />XHIBITS:Exhibits 0, and R are missing from the submittal. <br />Proof of Mailing is attached in the updated submittal. Proof of filing with the Clerk is included in the <br />updated submittal. A new copy of the submittal was delivered via certified mail as well, and a copy <br />of the return receipt will be delivered to the DRMS. <br />Exhibit S is incomplete as no proof has been provided that damage waiver agreements were provided to <br />any of the listed structure owners within 200 feet of the permit as required by Rule 6.4.19(x). <br />Included in the updated submittal is the proof of mailing to the structure owners within two -hundred <br />feet of the active mining area. A letter to all the property owners within 200 feet has been delivered <br />stating the intent of the Amendment and that no further mining will occur in the original North La <br />Poudre Mine area. It should be noted that the only structure within 200 feet of the non-active mine <br />IN. La Poudre Mine) is the GST Trust Property and a letter stating no further mining adjacent to their <br />property will occur and their structure is safe, has been delivered. <br />Rule 1.10.1(1) also states that "Where applicable, there shall be filed with any application for a 112 <br />Reclamation Permit amendment, attachment(s) map(s) and one (1) original and four (4) copies of the <br />application with the same content as required for an original application, except that the Applicant will not be <br />required to submit any information which duplicates applicable previous submittals. However, the Applicant <br />shall clearly describe where in the original application and supporting documents the information not <br />included in the amendment application, but necessary to render the amendment technically adequate, may <br />be found. Generally speaking, with a 17 year-old permit such as this, a new/revised set of all required <br />exhibits, especially maps, will be the easiest way to provide sufficient information to DRMS.
