PERMIT#:M-2010-084 <br /> INSPECTOR'S INITIALS: THM <br /> INSPECTION DATE:March 28,2017 <br /> OBSERVATIONS <br /> This inspection was conducted as part of the normal monitoring program established by the Colorado Division <br /> of Reclamation, Mining and Safety(Division).The Lost Cause Pond is located approximately 2 miles south of <br /> Rio Blanco County Road 26. The site is a 110c operation with 9.9 permitted acres, only 4.5 acres appeared to <br /> be affected during the inspection. Dean Mantle was the permit representative present during the inspection. <br /> A sign located at the main entrance and permit area boundaries were observed and in compliance with Rule <br /> 3.1.12. <br /> The site was inactive during the inspection. Various stockpiles of construction material were observed <br /> adjacent to the pond that was located in the middle of the area of disturbance. <br /> It appeared that the operation was in compliance with the approved mine plan. No problems or possible <br /> violations were observed during the inspection. <br /> In an effort to ensure that the financial warranty remains adequate to reclaim the site per the requirements of <br /> the approved reclamation plan, DRMS conducts routine reclamation cost estimates. DRMS staff has <br /> determined that the currently held reclamation bond in the amount of$7,000.00 is adequate to reclaim the <br /> site. <br /> Photos that show the conditions observed during the March 28, 2017 inspection are included at the end of <br /> this report. <br /> Responses to this inspection report should be directed to Travis Marshall at the Division of Reclamation, <br /> Mining and Safety, Grand Junction Field Office, 101 South 3rd Street, Room 301, Grand Junction, Colorado <br /> 81501, phone no. 970-241-2042. <br /> Page 2 of 4 <br />
