PERMIT#:M-i995-017 <br /> INSPECTOR'S INITIALS:CMR <br /> INSPECTION DATE: September 7,2012 <br /> OBSERVATIONS <br /> 1) The Division conducted a monitoring inspection of the site on 9/7/12. Present during the inspection <br /> was Mr. Lee Roy Roth, the operator. <br /> 2) The site is a small 110 sand and gravel operation that is in reclamation. Only a small area along the <br /> south side of the permit area where a small processed stockpiled material exists is the only area, which <br /> still requires reclamation.The post mining land use for the site is crop land.The lack of a permit sign <br /> and exposure of ground water were cited as possible problems in the inspection report dated <br /> November 21, 2006. Both possible problems have been abated. <br /> 3) The entire permit area was walked and inspected. No major noxious weed infestation was observed at <br /> the time of the inspection. The reclaimed area was found to be vegetated and stable. <br /> 4) Permit sign and boundary markers were properly displayed. <br /> 5) The current financial warranty in the amount of$5,650.00 is deemed adequate at this time. <br /> Page 2 of 4 <br />
