The <br /> following item is incomplete for the application to be processed: <br /> 1. Structure Agreements were not provided. See instructions enclosed for Structure Agreements. <br /> A list of your Permanent Man-Made Structures and a sample Structure Agreement are enclosed. <br /> Please provide proof of mailing to the structure owner, such as the certified or priority mailing <br /> receipt tracking number. <br /> Please return the required documentation to the Division before June 17, 2017 to complete the application <br /> for approval.
