Amazonite Mining, LLC <br />8057 S Vermejo Peak <br />Littleton, CO 80127 <br />April 5, 2017 <br />State of Colorado <br />Department of Natural Resources <br />Division of Reclamation, Mining and Safety <br />1313 Sherman St. Room 215 <br />Denver, Colorado 80203 <br />RE: Forrest Mining Project reclamation permit, 11' submittal <br />Hello Elliot, <br />Please find attached documents to be used for the Forrest Mining Project reclamation permit application. <br />Sense we last talked there has been some changes in the name of the applicant/operator. <br />I spoke with Dan Williams of Teller County planning 2 weeks ago and he voiced concerns that the Teller <br />County attorney may not view me as being able to personally "mine" on the operation unless I had a <br />more direct connection to the applicant. <br />To address this Jeffrey Forrest created a Limited Liability Company (Amazonite Mining) with the Company <br />members being Jeffrey Forrest, Betsy Forrest and myself. <br />Also attached is an Affidavit of Authority for Jeffrey Forrest to execute financial warranty documents. <br />hope it is filled out correctly. <br />Also attached are Amazonite Mining, LLC Resolutions with the Company members authorizing Jeffrey <br />Forrest to execute financial warranty documents and to have the title of Operator. And also Company <br />members authorizing myself to submit application materials and make permit commitments and to hold a <br />title of Field Representative. <br />A structure agreement regarding Forest Service Road 273 (6.3.12 Exhibit L) has executed by Jeffrey Forrest <br />and has been forwarded (attached is the postage receipt) to the Forest Service. I assume they will execute <br />it ASAP and they will forward a copy to you for the application attachment called for in Exhibit L. <br />Thank you again for your assistance.
