Preliminary review of the <br /> information received determined that the following items must be received before the Division can consider your <br /> application as being filed and technical review can begin: <br /> APPLICATION FORM: <br /> Lines 1.1 and 5.2 were left blank,and the"Applicant/operator or company name"given on line 1 does not match <br /> that used on the signature page(page 8). <br /> EXHIBITS: <br /> Exhibits Q, and R are missing from the submittal. <br /> Exhibit S is incomplete as no proof has been provided that damage waiver agreements were provided to any of the <br /> listed structure owners within 200 feet of the permit as required by Rule 6.4.19(a). <br /> DRMS is required to process amendments to existing applications in accordance with Rule 1.10.1(4)which states <br /> that"Applications for amendments shall be reviewed by the Board or Office in the same manner as applications for <br /> new Permits." <br /> Rule 1.10.1(1)also states that"Where applicable,there shall be filed with any application for a 112 Reclamation <br /> Permit amendment, attachment(s)map(s)and one(1)original and four(4)copies of the application with the same <br /> content as required for an original application,except that the Applicant will not be required to submit any <br /> information which duplicates applicable previous submittals. However,the Applicant shall clearly describe where in <br /> the original application and supporting documents the information not included in the amendment application,but <br /> necessary to render the amendment technically adequate,may be found. Generally speaking,with a 17-year-old <br /> permit such as this, a new/revised set of all required exhibits, especially maps,will be the easiest way to provide <br /> sufficient information to DRMS.
