Dear Mr. West,
On behalf of Sunrise Mining, LLC, R Squared, Inc. is submitting an amendment to
112d-1 Application, May Day Idaho Mine Complex, File No. M-1981-185. The amendment is
submitted to comply with Board Order issued on December 12, 2011 to permit the Operator to
commence mining. The provision addressed prior to 2017 includes:
• Provision 1-"stabilization and reclamation of the new access road;
• Provision 2-"stabilization and reclamation of Little Deadwood Gulch at the Chief Portal
and May Day 2 areas:
• Provision 3-"stabilization of the collapsed adit at the May Day 1 area;
• Provision :
The submitted amendment addresses the following Provisions:
• Provision 4-"provide adequate information for ambient surface and ground water
quality..."
• Provision 5-"satisfy the requirements of Rules 6.4.4, 6.4.5, 6.4.6 and 6.5;
• Provision 6-"design specifications certified by a licensed professional engineer for all
Environmental Protection Facilities ...satisfies the specific requirements of Rules 7.3 and
7.4 ..."; and
• Provision 7-"an updated Exhibit L and financial warranty..."
The amendment addresses the Board provisions and includes the following:
• One bound original permit amendment and three unbound copies;
• A CD containing the electronic amendment contents;
• A comprehensive water quality summary including laboratory data sheets;
• Certified receipt documenting notices have been submitted to the Board of County
Commissioners, and the La Plata County Soil Conservation District
