Environmental Protection Specialist Colorado Department of Natural Resources Division of Reclamation, Mining and Safety Office of Mined Land Reclamation 1313 Sherman Street, Room 215 Denver, Colorado 80203 Re: Permit No. M-1980-244; Cripple Creek & Victor Gold Mining Company (°_CC&V�); Cresson Proiect; — Follow Up Report to High Grade Mill (HGM) High PH Thickener Overflow tank overtoppinq event, February 11, 2017 Dear Mr. Cazier: In accordance with Rule 8, Section 8.2.3 of the Hard Rock, Metal and Designated Mining Operations Regulations (2015), CC&V hereby provide the Division with a formal report regarding the 2500 gallon process water release that occurred at operation on 11"' February 2017. Miscommunication during event reporting between operations and environment personnel resulted in an incorrect tank reference and location initially being reported to the Division. The correct tank reference is the High pH Thickener overflow tank and is shown as Figure 1, within Attachment 1 and Figure 2 in Attachment 2. The following points addresses the requirements as outlined within Rule 8, Sub section 8.2. a. Actions taken to respond to and correct the emergency situation or condition At approximately 2:00am, 2500gal of High pH water, measuring 200ppm WAD CN, overtopped the High pH Thickener overflow tank at the High Grade Mill Facility. Adjustments were made to the processing circuit to prevent the tank from overflowing further. Flow from the circuit feeding the affected area was shut off until control could be regained. The full Mill circuit was also shut down until control of the plant could be regained. Communication was made to the Mill General Foreman, Process Superintendent and the Environmental representative on call.
