LaSalle St. Suite 3100 <br />Chicago IL 60605 <br />RECEIVED <br />FEB 0 6 2011 <br />DIVISION OF RECLAMATION <br />MINING AND SAFETY <br />L <br />RE: Request for an extension to the corrective action date of 2/10/2017 for the corrective action <br />specified in the 12/7/2016 Inspection Report, Items (a), (b) (c), Daniels Sand Pit #2, M1973- <br />007SG. <br />Dear Mr. Cazier: <br />Transit Mix Concrete Co., on behalf of Continental Materials Corporation, requests an extension <br />to the corrective action date of February 10, 2017, stated in your December 12, 2016 Inspection <br />Report. The purpose of the extension request is to allow time to complete the corrective action <br />items (a), (b) and (c), specifically: <br />• "(a) Place additional material on the outside face of the southwest embankment to <br />achieve a slope no steeper than 3H:1 V;" <br />Task Extension Date Request: <br />Due to adverse weather related construction delays, additional time is needed. We <br />estimate we will need an additional 45 days for proper placement of fill. The requested <br />corrective action date for completion of the fill is March 27, 2017. <br />• "(b) Work to establish a vegetative cover to reduce erosion potential;" <br />Task Extension Date Request: <br />We believe drill seeding, followed by applying weed free straw mulch, is the most <br />effective way to establish erosion controlling vegetation. This task should be done when <br />the ground is not frozen or muddy. Therefore, this task should be done in the spring <br />rather than during winter months. We request an extension of 90 days to complete <br />seeding and mulching (May 11, 2017). <br />• "(c) Conduct a geotechnical investigation for the purpose of assessing the slope stability <br />of this embankment." <br />Task Extension Date Request: <br />We request 45 days plus 15 days for installation of the piezometers (April 11, 2017). The <br />extension will allow the piezometers to be installed once slope backfilling is complete. If <br />
