Prequal. expiration date: 4/30/17 <br /> Proposal for Project No. (s) NHPP1602-142 <br /> I, the undersigned bidder for the project(s) listed above, hereby offer this proposal to the Chief <br /> Engineer, Department of Transportation, 4201 East Arkansas Ave., Denver CO 60222. <br /> I UNDERSTAND the bidding and award for this project(s) shall be in accordance with the rules that <br /> implement the "Construction Bidding for Public Projects Act", Sections 24-92-101 et seq. , C.R.S. <br /> I FURTHER UNDERSTAND that if this proposal is accepted, the Invitation for Bids, Proposal, Plans, <br /> Standard Specifications, Supplemental Specifications, Special Provisions, Contract Form, and Contract <br /> Bonds shall be part of the complete contract. <br /> I FURTHER UNDERSTAND that failure to include this Form #85 in the proposal will cause this proposal to <br /> be rejected. <br /> IN PREPARING THIS PROPOSAL, I HAVE: <br /> - examined the Plans, Specifications, and the site of the proposed project and I am satisfied as to the <br /> conditions to be met and the work to be accomplished. <br /> - considered any revisions to the Plans and Specifications which were made after advertisement. Number <br /> of revisions: 1 <br /> IF THIS PROPOSAL IS ACCEPTED AND I AM AWARDED THE CONTRACT, I AGREE TO: <br /> - execute a Contract and Bonds on forms provided by the Department of Transportation within 15 calendar <br /> days after the date of award. I understand that failure to do so will be just cause for cancellation of <br /> the award and forfeiture of the proposal guarantee. <br /> - commence work within 20 calendar days (or other time as specified in the Notice to Proceed) after the <br /> award date. I will complete the work in accordance with the time stated in the project Specifications. <br /> - accomplish the work on the project in accordance with the Plans and Specifications, under the <br /> direction and according to the instructions of the Chief Engineer or, authorized representative, at the <br /> unit prices named in this proposal. <br /> - perform any increased or decreased quantities of work at the unit prices named in the Proposal, except <br /> for alterations as provided for in the Specifications. I understand the Bid Schedule of quantities shown <br /> in this proposal is approximate and subject to change and that the Department may, in its sole <br /> discretion, unilaterally increase the funds available under the Contract if the quantities increase. <br /> - furnish all labor, machinery, equipment, materials, and supplies necessary to accomplish the work on <br /> the project. <br /> - provide adequate compensation insurance to protect my employees working on the project. <br /> ACCEPTANCE OF FUEL COST ADJUSTMENTS: <br /> Bidders have the option to accept Fuel Cost Adjustments in accordance with the Revision of Section 109 - <br /> Fuel Cost Adjustment. To accept this standard special provision, the bidder must fill in an "X" next to <br /> "YES" below. No Fuel Cost Adjustment will be made due to fuel cost changes for bidders who answer "NO". <br /> If neither line is marked, the Department will assume the bidder rejects Fuel Cost Adjustments for this <br /> project.
