.2.04.13 ANNUAL RECLAMATION REPORT <br />(1) By February 15, or other such date as agreed on, each permittee shall file an annual reclamation <br />report coverin, the previous calendar year for all areas under bond. The report shall include, but not be limited to <br />test, discussion and maps which address. <br />(a) The name and address of the permittee and permit number; <br />(b) Location and number of acres disturbed during that year; <br />( c) Location and number of acres backfilled and graded during that year; <br />(d) Location and number acres topsoiled during that year; <br />(e) The species, location and number of acres of vegetation planted during that year, includi119 <br />any augmented seeding or cultural practices; and <br />(f) Location, number of acres and date of planting for all previously revegetated areas. <br />(2) The permittee may provide additional monitoring information as required by the approved permit. <br />RESPONSE <br />TCC will submit the required Annual reclamation Report to the Division by March 15 of each year in <br />40 accordance with Rule 2.04.13. <br />MR 97 -154 2.04 -52 Revised 10/02/97 <br />
