PERMIT#:M-1992-051 <br /> INSPECTOR'S INITIALS:TC1 <br /> INSPECTION DATE:December 22,2015 <br /> OBSERVATIONS <br /> This inspection was conducted at the request of the new Operator to discuss permit requirements, including <br /> reclamation plans and phased bonding. The Operator (Langston Concrete) was represented during the <br /> inspection by Messrs. Mike and Zac Langston. The Florence Sand & Gravel Pit site entrance is located <br /> approximately one quarter mile south of the S. Union St railroad crossing in Florence. This is a 112c sand and <br /> gravel mine. The site was active at the time of the inspection. <br /> Availability of Records: We discussed discrepancies in the permit boundary maps and determined a technical <br /> revision (TR) was required to clarify the permit area boundary. Subsequent to this inspection and prior to the <br /> release of this report,TR-01 was approved by the Division to clarify the permit boundary. <br /> Backfilling and Grading: The northeast portion of the pit was visited (see Photo 1). It was agreed that the <br /> highwalls needed to be stabilized and flattened without excavating more material. Mr. Langston indicated they <br /> would bring backfill in from elsewhere on site. <br /> Financial Warranty: This inspector explained the permit at the time of the inspection allowed a maximum of 35 <br /> acres of disturbance at one time, but permit allowed 77.4 acres. <br /> Fish and Wildlife: No impact on wildlife was observed. <br /> Hydrologic Balance: Some standing water was observed in the pit (see Photo 2). It was unclear if this was <br /> groundwater or captured stormwater. Mr. Langston committed to keeping the pit floor at least 2 feet above <br /> the normal high groundwater elevation as required. <br /> Off-site Damage: The operation appeared to be confined to the permit boundary. <br /> Roads: Haul and access roads did not appear to be a source of sediment that could be tracked offsite. <br /> Revegetation: No revegetated areas or noxious weeds were observed. <br /> Support Facilities On-site: Several screens and conveyors were observed on site (see Photo 3). <br /> Signs and Markers: The permit sign was properly posted (see Photo 4) and boundary fences were observed on <br /> the north side (see Photo 5). An access road serves to delineate the south side (see Photo 6). <br /> Permit Stipulations: There are no permit stipulations. <br /> Page 2 of 4 <br />
