EFCI - Southfield Mine Permit No. C-81-014 <br />NOV No. CV -2004-003 <br />Issued by: Division of Minerals and Geology <br />Date: September 13, 2004 <br />Description: Due to previous revision of the approved seed mix <br />for the Corley parcel, the incorrect seed mix was planted. <br />Abatement: Following approval of the DMG, interseeding of <br />Beardless Bluebunch Wheat grass and Indian Rice grass was <br />seeded to complete the approved grass mix. <br />Resolution: An Assessment Conference was held 11-30-04. <br />Seriousness and Fault were reduced to a total of $500.00 was <br />assessed and paid. NOV CV -2004-003 was abated as of October <br />8, 2004. <br />EFCI - Southfield Mine Permit No. C-81-014 <br />NOV No. CV -2013-002 <br />Issued by: Division Reclamation Mining & Safety <br />Date: January 28, 2013 <br />Description: A partial obstruction occurred in monitor well MW - <br />NW 20 feet above the bottom of the well which restricted well <br />probe & monitoring from the bottom portion of the well. <br />Abatement: On 03-01-2013. an assessment fee was set at <br />$1,000.00 and the assessment was paid. <br />Resolution: Concerning Step 1 of the NOV: Effective March 25, <br />2014 the DRMS modified the NOV to include: "- - - or provide <br />information confirming that the well is currently functional for <br />the intended purpose". On April 30, 2014 a well test was <br />performed on the well which proved the well was open to water <br />flow. (A previous well test, April 17, 2012, also proved the well <br />was open to water flow). Final resolution to the NOV is currently <br />pending. <br />Step 2: Consultants for EFCI are processing the monitor well <br />permit applications and approval from the DWR is pending. <br />Revised 05-2014 <br />2.03 - 25b RN -06 <br />
