CERTIFIED MAIL <br /> RETURN RECEIPT NO.7015 0640 0007 7571 8363 <br /> January 20, 2017 <br /> Division of Reclamation, Mining& Safety AGGREGATE <br /> 1313 Sherman Street,Room 215 INDUSTRIES <br /> Denver,CO 80203 <br /> Re: Permit No. M-1977-141 -Lyons Quarry-Annual Report <br /> Ladies and Gentlemen: <br /> Enclosed is Aggregate Industries' check in the amount of$791.00 together with the annual report <br /> form for the above-referenced permit. On February 16,2011,the Division approved Aggregate <br /> Industries' request for a temporary cessation. The first five-year term of the cessation expired <br /> February 16, 2016. As Aggregate Industries does not anticipate any future mining activities at <br /> the site,the Division advised that it considers the site active and that reclamation must proceed. <br /> As noted in the Division's report of its March 2, 2016 inspection, conditions at the Lyons Quarry <br /> site mostly remain unchanged since a previous inspection that was conducted following the flood <br /> event that occurred on South St. Wain Creek in September 2013. The site was significantly <br /> impacted by that event and the site access road and bridge were washed out. Boulder County <br /> anticipates restoring access sometime in 2017. Until such time as access is restored,the only way <br /> to access the site is by foot;therefore,no reclamation activity has been progressed at the site. <br /> Aggregate Industries has proposed to revise the currently approved reclamation plan and has <br /> submitted the proposed plan to the Division as a technical revision request. The revision is under <br /> review by the Division as Technical Revision No. 2. Following approval by the Division of the <br /> Technical Revision,Aggregate will proceed with reclamation of the site when site access is <br /> restored. <br /> Since there has been no mining or reclamation activity,the most recent Google Earth aerial <br /> photography from October 9, 2015 is an accurate reflection of the current site status and is <br /> enclosed as the annual report map. <br /> If you have any questions,please contact me at(970)396-5252. <br /> Very truly yours, <br /> � M 1 <br /> Connie N.
