The <br />inspection was conducted by Clayton Wein and Jim Stark from the Division of Reclamation, Mining and Safety <br />(the Division). No company representative was present for the inspection. The weather was cloudy with a <br />temperature of 40 degrees farenheight. The loadout was covered in several inches of snow. There were no <br />activities occuring during the inspection. The loadout is currently inactive and in reclamation. Most of the <br />structures at the site have been demolished. SO-1 was issued by the Division on January 13, 2017. <br />HYDROLOGIC BALANCE - Rule 4.05 <br />Drainage Control 4.05.1, 4.05.2, 4.05.3; Siltation Structures 4.05.5, 4.05.6; Discharge Structures 4.05.7, 4.05.10; <br />Diversions 4.05.4; Effluent Limits 4.05.2; Ground Water Monitoring 4.05.13; Surface Water Monitoring 4.05.13; <br />Drainage – Acid and Toxic Materials 4.05.8; Impoundments 4.05.6, 4.05.9; Stream Buffer Zones 4.05.18: <br /> The site was completely covered in snow at the time of the inspection. The sediment pond and the dug- <br />out pond were covered with snow. Both ponds were stable and there were no indications of erosion. There was <br />no discharge from the ponds at the site. The ditches had snow in them at the time of the inspection. There was no <br />flow through the ditches. <br /> <br />ROADS – Rule 4.03 <br />Construction 4.03.1(3)/4.03.2(3) <br />Drainage 4.03.1(4)/4.03.2(4) <br />Surfacing and Maintenance4.03.1(5) and (6)/4.03.2(5) and (6) <br />Reclamation 4.03.1(7)/4.03.2(7): <br /> The main road at the loadout was covered in snow. There were no ruts or erosional issues noted. <br /> <br />SIGNS AND MARKERS – Rule 4.02: <br /> All of the required signs and markers were in place. SO-1 was issued on January 13, 2017. The name <br />Terror Creek, LLC was changed to Oxbow Mining, LLC. Part of the requirements of SO-1 include that the names <br />on the mine ID signs will need to be changed to identify the current permittee.
