December 30, 2016
Mr. Jonathan Ma
Rocky Mountain Resources
9301 Wilshire Blvd, Suite 312
Beverly Hills, CA 90210
Re: Requested Structure Agreement
Mr. Ma:
REyE1VED
jAN 11 2017
Division
of Recl a ation,

iv'nning Y
The purpose of this letter is to allow your firm to complete the Structure Agreement Application as required by the
Colorado Division of Reclamation, Mining & Safety (Division). Per the Division's rules, the Glenwood Springs Electric
System, through the City of Glenwood Springs, is providing this notarized letter regarding our infrastructure located
within 200 feet of the active work area at the Mid -Continent Limestone Quarry, located near Glenwood Springs,
Colorado, that may be subject to damage from mining operations.
Those facilities include the 1000 kva 480/277 transformer, Number 3289 and associated three 200:5 current transformers
and a 13 terminal meter can that serves the main building; the rock crusher is served by transformer #3288 1000kva
4801277, three 300:5 current transformers and a 13 terminal meter can. These facilities included the associated
infrastructure supporting the service connections.
In 2010 the CaIX Minerals, LLC (CaIX) requested permission to place transformer #3288, which was stored in the City's
inventory for the use in the event transformer #3289 failed, in service for the rock crusher. CaIX was granted permission
with the understanding the rock crusher would be out of service if #3288 was needed to replace #3289 until a replacement
transformer was purchased.
If the new operator of the mine — Rocky Mountain Resources (RMR) — feel the rock crusher transformer will be utilized in
similar mine operations compared to Ca1X's operations, the City recommends the transformer be changed out with a
transformer properly sized for the rock crusher and transformer #3288 be brought back into the City's inventory. Due to
the fact CaIX purchased two 1000kva transformers the City does not believe additional funds from RMR will be
necessary. Two transformers were purchased, two transformer will be in service but the City will gain a transformer large
enough to replace either in the event one fails.
Kindly let me know if you have additional questions.
Sincerely,
Zt= "' 'I ,
Robin Millyard
Public Works Director
STATE OF COLORADO )
)ss.
COUNTY OF GARFIELD )
The foregoing was acknowledged before me this 301 day of December, 2016, by Robin Millyard as Public Works
Director of the City of Glenwood Springs, Colorado.
Witness my hand a
p{tyHAFLENGER
BLIC
Y Sabir'GBI.ORADO y
M commission a fres: No Public:
NOTARY ID #20044007023
My Commk8im Expires February 27, 202Q Address: io l td. ! 17
C+1ehuxL 4r,* (b
