EXHIBIT A - Legal Description and Location Map (Rule 6.3.1): <br />3. The Application did not provide an Exhibit A Location Map. In accordance with Rule 6.3.1(3), <br />please submit a map that identifies the affected lands, existing and proposed roads or access routes to <br />be used, and adjacent surface owners. Please note, several maps submitted with the application <br />(following the last page of the Exhibits) contain this requested information, but are identified as <br />Exhibit E Maps. <br />The maps have been re -labeled as appropriate for a 110 permit. The location map identifies <br />the affected land. <br />EXHIBIT G — Source of Legal Right -to -Enter (Rule 6.3.7): <br />4. The Applicant has stated that Stanley Laybourn is the surface/subsurface owner of the affected <br />lands. The application includes a commissioner resolution and a lease agreement between <br />Washington County and Stanley Laybourn. In accordance with Rule 6.3.7, please provide a copy of <br />the deed, abstract of title, or current tax receipt showing Stanley Laybourn is the landowner. <br />I have provided a copy of the deed showing title to the land affected in the name of Stanley <br />Laybourn. <br />EXHIBIT I - Proof of Filing with County Clerk (Rule 6.3.9): <br />5. Any changes or additions to the application on file with the Division must also be reflected in the <br />public review copy.
