3 <br /> <br />7. Check dams on roads need to be monitored for maintenance. <br />8. Ditch D18 requires maintenance. <br />9. The top of the dam at Pond 8 is sparsely vegetated and requires hydroseeding while precipitation <br />events are likely. <br /> <br />OCTOBER 2016 MAINTENANCE ITEMS <br />10. At Pond 9, wattles require repair and hydroseeding is needed. <br />11. Pond 6 exhibited some erosional features requiring attention. <br />12. Cut culvert C1, identified in previous reports had been addressed and at this time requires <br />backdressing. <br />13. Slope on The Knob to the north of culvert C3 has several erosional features that could be <br />effectively mitigated with branches, encouraging self healing. <br />14. Rilling on road above pond 5 requires repair and stakes should be removed as they present a <br />wildlife hazard. <br />15. Pond 5 erosional features above check dam and gully below check dam. <br />16. Several check dams and sumps around the site require cleaning. <br />INSPECTION: <br />SIGNS AND MARKERS Rule 4.02 <br />o Disturbed area boundary markers observed were intact and appeared properly placed. <br />o The mine site identification sign at the entrance to the Lorencito site appeared intact, displaying <br />all required information: mine name, operator name, address, phone number and DRMS permit <br />number and contact information. <br /> <br />ROADS Rule 4.03, <br />Construction 4.03.1(3), 4.03.2(3); Drainage 4.03.1(4), 4.03.2(4); Surfacing and Maintenance 4.03.1(5) <br />and (6) 4.03.2(5) and (6). <br />o The roads into and around the facility were inspected. Some road sections require grading, <br />however overall it appeared that the operator was maintaining roads in good working order <br />with the exception of: <br />a. Rilling on road above pond 5 requires repair and stakes should be removed they present a <br />wildlife hazard, (maintenance items nu 14). <br />o No fugitive dust was being generated. <br />o Cut culvert C1, identified in previous reports had been addressed and currently requires <br />backdressing. <br />o Culverts observed appeared to be functioning properly. <br /> <br />HYDROLOGIC BALANCE- Rule 4.05 <br />General Requirements 4.05.1; Divergence and Conveyance of Overland Flow 4.05.3; Diversions 4.05.4; <br />Sediment Control Measures 4.05.5, 4.05.6; Discharge Structures 4.05.7; Impoundments 4.05.9: <br /> <br />Ponds 5, 6 and 9 and were inspected and holding water. Generally embankments and spillways appeared <br />in good condition. Some erosional features exist on pond 5, The Knob, the approach to the shrub plot <br />from the south east where bales and wattles require repair and the slope west of The Knob above the <br />sediment dump. Treatment of these features includes filling gullies with branches and installing straw <br />bales or wattles. Erosional features on the southeast face of The Knob are most likely due to a past ditch <br />breach above. The west side of The Knob is slated for additional erosion control. <br />o Pond 9 or, the North Pond, held water but was not discharging at time of inspection (photo 1). <br />The armored channel directing flow to the pond and perimeter ditch (D3) around fill 9 appeared <br />to be in good repair and functioning adequately. <br /> <br /> <br />
