The weather was clear and the site was dry with a small amount of snow on the
ground during the on-the-ground inspection of the site. No activities were on-going during the inspection. The
majority of the structures at the site have been demolished and the site is currently inactive and in reclamation. An
SO (SO-1) application is currently under review by the Division.
HYDROLOGIC BALANCE - Rule 4.05
Drainage Control 4.05.1, 4.05.2, 4.05.3; Siltation Structures 4.05.5, 4.05.6; Discharge Structures 4.05.7, 4.05.10;
Diversions 4.05.4; Effluent Limits 4.05.2; Ground Water Monitoring 4.05.13; Surface Water Monitoring 4.05.13;
Drainage – Acid and Toxic Materials 4.05.8; Impoundments 4.05.6, 4.05.9; Stream Buffer Zones 4.05.18:
The sediment pond was damp with a small amount of snow cover and no evidence of recent discharge and the
valve of the primary discharge pipe was closed. The dug-out pond was dry with no discharge at the time of the
inspection. Embankments were well vegetated and stable with no evidence of erosion and no issues noted.

Ditches and culverts throughout the Loadout facilities area are clear with no issues identified.

SUPPORT FACILITIES - Rule 4.04:
Facilities remaining on-site are the concrete pad from the shop and the office. A portion of the crushing and
screening plant and the truck scale and scale house remain on site. Footers and wing walls remain.

The site is stable with no issues noted at the time of the inspection.

SIGNS AND MARKERS – Rule 4.02:
All signs and markers were in place and in compliance at the time of the inspection. It is noted that the operator
has submitted an SO application (SO-1) to change the name from Terror Creek, LLC to Oxbow Mining, LLC. As
a part of the requirements of the SO, the names on the Mine ID signs will require revision to identify the current
permittee once the SO-1 application has been approved.
