The inspection was conducted by Tabetha Lynch with the Division of
Reclamation, Mining and Safety. Miranda Blomquist with MCM was present during the inspection. The weather
was cloudy and warm. Cows were grazing on the site at the time of inspection and some fields were recently
tilled. Reclamation activities are occurring at the site. A salvage crew had begun work near portal 5A/6. The
Division received notice on November 10, 2016 from MCM removing the Williams Fork Mine from Temporary
Cessation Status. The return to active status due to reclamation activites increases the inspection frequency by the
Division to two partial inspections and one complete inspection each quarter. The change in status also returns the
site to the orginal permit requirements with regard to hydrologic monitoring.
AVAILABILITY OF RECORDS – Rule 5.02.4(1):
The mine records were checked on site. See the end of this report for details.

HYDROLOGIC BALANCE - Rule 4.05
Drainage Control 4.05.1, 4.05.2, 4.05.3; Siltation Structures 4.05.5, 4.05.6; Discharge Structures 4.05.7, 4.05.10;
Diversions 4.05.4; Effluent Limits 4.05.2; Ground Water Monitoring 4.05.13; Surface Water Monitoring 4.05.13;
Drainage – Acid and Toxic Materials 4.05.8; Impoundments 4.05.6, 4.05.9; Stream Buffer Zones 4.05.18:
All ponds were dry at the time of inspection. Spoil spring, discharge point SP1, was holding water behind
the V-notch but not discharging. The water level was about an inch below the point of discharge. Cattail will be
cleared once water level decreases. Culverts all appeared functional. Culverts under train tracks were clear.
