PERMIT #: M-1980-244 <br />INSPECTOR’S INITIALS: TC1 <br />INSPECTION DATE: September 20, 2016 <br /> <br /> <br />Page 3 of 7 <br />sediment (no standing water) was observed I the second sump. Newly placed liner was observed between the <br />HGM and the adjacent stormwater pond was observed (see Photos 3 and 4). <br />SGVLF Inspection: Water levels were observed on the four High Volume Solution Collection System (HVSCS) <br />transducer displays and the nearby piezometer. The following water levels were recorded from the SGVLF <br />HVSCS Transducers: 88301 @ 58.58; 88303 @ 58.33; 88305 @ 58.70; 88307 @ 58.38; Piezometer @ 61.42. The <br />relative water levels were within expected norms. <br />An area of ponded solution approximately 12 feet by 12 feet was observed near the crest of the SGVLF access <br />road from the SGVLF ADR. This by far exceeds the 3 ft x 3 ft limit in the permit’s wildlife protection plan. The <br />ponded solution was later mitigated, and therefore not cited as a problem. However, the ponded solution <br />problem appears to be happening with a much greater frequency than observed by the Division in the past. Mr. <br />Munroe indicated it is the responsibility of CC&V’s Process staff to inspect the VLFs for ponded solution. The <br />Division recommends additional inspections be performed to ensure the frequency of this recurring problem is <br />significantly reduced. <br /> <br />AGVLF Inspection: The Division visited the east side of the Phase V AGVLF to determine if the edge-of-liner signs <br />missing during the Division’s August 18, 2016 inspection had been replaced. The signs were still missing, but <br />not cited as a problem in this report as this was cited as a problem in the report for the aforementioned <br />inspection. <br /> <br />Buckley Annex/Bulk Emulsion Facility & Explosives Storage Area: These two facilities were inspected for the <br />presence of designated chemicals and for general housekeeping with CC&V’s lead blaster, Mr. Marc King. No <br />problems were observed. <br /> <br />SGADR Area Inspection: Mine representatives showed this inspector progress made on addressing the erosion <br />observed previously in the area NE of the SGADR/ADR2 where process solution pipe line supports had failed. <br />New pipe supports had been poured (see Photo 5) and drainage ways from the ADR to the VLF toe trench had <br />been cut under the pipe (see Photo 6). <br /> <br />Summary: <br />1. With support from Jeff Campbell, the Division will inspect the leak detection system in November 2016. <br />2. CC&V should increase the inspection frequency on the VLFs to significantly reduce the number of <br />occurrences of solution ponding exceeding the 3 ft by 3 ft maximum allowed area in the permit’s wildlife <br />protection plan. <br />3. CC&V must address the change in the 80 percent reporting limit for the SGVLF in a future TR. This can <br />be combined with another TR, but should be coordinated with the Division prior to submitting it to be <br />assured the two (or more) items are compatible for a single TR request. <br />4.
