Resources <br /> November 01, 2016 <br /> Mr. Michael Cunningham <br /> Colorado Division of Reclamation Mining and Safety <br /> 1313 Sherman Street/ Room 215 <br /> Denver, Colorado 80203 <br /> Reference: Annual Report and Map: Permit M-1977-410 <br /> Cross Gold Mine: Boulder County, Colorado (Caribou) <br /> Dear Michael: <br /> Please find attached our annual report for the Cross Gold Mine, Permit Number <br /> M-1977-410 for the year November 04, 2015 through November 03, 2016. 1 have <br /> also attached the fee due for this permit in the amount of$ 259.00. <br /> The mine continued to be closed during this period. I worked seven days a <br /> week, year round at the Cross Mine and Caribou District. I completed the <br /> property maintenance, asset preservation, property safety, daily inspections of <br /> the.powder magazines for the ATF, and maintained the roads. I maintained all <br /> the permits, including the air permit, Stbrmwater Permit and the mine water <br /> discharge permit. The mine water discharge system, requires work by me daily. I <br /> lime the ponds, shovel lime, maintain the pH, take daily readings which include <br /> temperature readings, pH, flows, and monitoring and recording the upper Coon <br /> Track Creek flow weir. I take bi-monthly water samples in accordance with our <br /> WQCD / CDH permit, submit them to SGS Accutest Labs in Wheatridge, Colo. I <br /> submit monthly the reports, DMR's and statistics to the WQCD..I also take the <br /> quarterly Chronic Wet test samples and deliver them to Sea Crest Labs in <br /> Louisville, Colorado. I then file all the required reporting. I maintain and <br /> extensive data base at the mine office and at my home office.
