Review of this application determined the following items must be <br /> received before the Division can consider the application as being submitted and the technical review can <br /> begin. Please respond to this Incompleteness Notice with the requested additional/updated information on <br /> permit replacement pages and/or exhibits and summarize each response in a cover letter titled <br /> "Incompleteness Response; M-2016-070". <br /> APPLICATION FORM <br /> �►1. The Applicant provided different coordinates for the Primary Mine Entrance Location listed on page <br /> 2 of the application compared to the coordinates provided in Exhibit A. After further review of these <br /> locations, neither of them identify the location along County Road U where the access road is <br /> located. Please revise these coordinates to reflect the correct location of the mine entrance and <br /> submit a new Page 2 of the application and Exhibit A. <br /> 2. The Applicant did not list a complete mailing address for the Applicant/Operator Correspondence <br /> Information on Page 3 of the application. Please provide the Street/P.O. Box information and submit a <br /> new Page 3 of the application. <br /> EXHIBIT O—Owners(s) of Record of Affected Land (Surface Area) and Owners of Substance to <br /> be Mined (Rule 6.4.15): <br /> 3. The Applicant has listed one induvial as an adjoining landowner within Exhibit O. Please verify that <br /> this is the only adjoining landowner within 200 feet of the proposed permit boundary. It appears, <br /> based on an Exhibit C Map, that there is at least one additional landowner west of where the access <br /> road intersects County Road U.
