PERMIT #: M-1977-022 <br />INSPECTOR'S INITIALS: SJM <br />INSPECTION DATE: October 19, 2016 <br />OBSERVATIONS <br />This was a normal monitoring inspection of the Pit No. 3, DRMS File# M1977-022 operated by Old Castle <br />Southwest Group, dba Uniting Companies of Mesa County. The site is located about 1mile west of Paonia, <br />Colorado in Delta County. I, Stephanie Mitchell of the Colorado Division of Reclamation, Mining and Safety <br />conducted the inspection. Mr. Jason Burkey of United Companies accompanied me on the inspection. <br />This is a 9.00 acre 110c mining operation for rock, sand and gravel. A mine identification sign was in place at <br />the entrance to the mine site. Permit boundaries were all clearly marked. The site is stable and no <br />disturbance was observed outside of the approved permit boundaries. <br />The Division currently holds a financial warranty amount of $4385.00 for this site. The Division will review the <br />reclamation costs for this site in order to ensure the financial warranty reflects the actual current cost of <br />fulfilling the requirements of the Reclamation Plan. The Operator will be notified if the Division determines <br />that the current financial warranty amount is insufficient as a separate cover. <br />Material is being stockpiled in the center of the pit and is in compliance with the mining plan. In addition, <br />little or no weeds appeared to exist on this site, although there were some Canadian thistle and Russian <br />Knapweed, which had been sprayed earlier this spring. <br />No fuel was being stored at the mine site. No equipment was at the mine site. <br />The city of Paonia has constructed a bike path in the northeastern portion of the permit area. In 2004, the <br />Operator submitted and then withdrew a Technical Revision (TR02) to change the reclamation plan. At this <br />time the bike path is not approved. The Operator will have to submit a Technical Revision to change the <br />reclamation plan and include a bike path in the permitted area. This is noted as a problem in the beginning of <br />this inspection report. <br />All other activities and completed work at the time of inspection appeared to follow the guidelines and <br />regulations set forth by the state and in the mining and reclamation plans for the pit. <br />Responses to this inspection report should be directed to Stephanie Mitchell at the Division of Reclamation, <br />Mining and Safety, Grand Junction Field Office, 101 South 3rd Street, Room 301, Grand Junction, Colorado <br />81501, phone number (970) 242-5025. <br />Page 3 of 6 <br />
