PERMIT #: M-2013-013 <br />INSPECTOR'S INITIALS: SJM <br />INSPECTION DATE: October 4, 2016 <br />OBSERVATIONS <br />This was a normal monitoring inspection of the Cook Gravel Pit; DRMS file No. M-2013-013 operated by Delta <br />County. This is a162.53 acre 112c mining operation located approximately 3 miles southwest of Delta, <br />Colorado in Delta County. I, Stephanie Mitchell of the Colorado Division of Reclamation, Mining and Safety <br />(Division) conducted the inspection. Mr. Larry Record and Mr. Bob Basher of Delta County accompanied me <br />on the inspection. <br />The appropriate mine entrance sign was in place. The permit boundaries were all clearly marked with t- posts. <br />The current disturbance is located in the southeastern portion of the permit area and is approximately 3.08 <br />acres in size, and moving to the west. The excavation is about 10 to 12 feet deep with a vertical highwall with <br />an eastern aspect. Topsoil has been stripped and is located in the southern part of the site and is stabilized <br />with vegetative cover. No erosion or sedimentation issues were observed. <br />All other activities and completed work at the time of inspection appeared to follow the guidelines and <br />regulations set forth by the state and in the mining and reclamation plans for the pit. <br />Page 2 of 4 <br />
