This reservoir is located <br /> in the SW1/4 and the NW1/4 of the SE1/4 of Section 9, Township 2 South, Range 67 West of the 61' P.M. <br /> and is a part of the Hazeltine Mine gravel pit (DRMS M-2004-031). Hazeltine Mine Reservoir was lined with a <br /> soil bentonite slurry wall. The slurry wall leak test initially commenced December 4, 2016. Site inspection <br /> was performed by our office on December 10, 2015 to review site conditions and discuss the leak test plan. <br /> After the initial leak test starting date it was discovered that the Parshall flume measuring flow entering <br /> the mine from Bull Seep has been installed backwards. On January 14, 2016 the flume installation was <br /> corrected. Therefore the slurry wall leak test start date was changed to January 14, 2016. The leak test <br /> concluded on April 14, 2016. <br /> Your report received May 9, 2016 provides the liner summary and leak test result. The test data provided <br /> indicates that the reservoir has been lined to the design standard referenced in the August 1999 State <br /> Engineer Guidelines for Lining Criteria for Gravel Pits (1999 SEO Guidelines). Meeting the design standard <br /> requires that during reservoir operations all water inflows and outflows for the liner perimeter <br /> enclosed area be accounted for on a monthly basis. <br /> With this liner approval, the Hazeltine Mine Reservoir is now classified as a lined reservoir in accordance <br /> with the 1999 SEO Guidelines. Water shall not be impounded in the reservoir except pursuant to lawful <br /> diversions allowed by statute or decree. At all other times, all inflow of water into the reservoir from any <br /> source, including precipitation and ground water inflows shall be removed to prevent illegal storage of <br /> water.
