PERMIT#: M-2011-021 <br /> INSPECTOR'S INITIALS:TCW <br /> INSPECTION DATE: October 18,2016 <br /> OBSERVATIONS <br /> This was a routine monitoring inspection conducted by TC Wait of DRMS. Jeff and PJ Chenoweth were present <br /> during the site visit representing the Operator. The site is located about 28 miles southeast of Springfield one <br /> mile west of CR 45 on CR G. The site was last inspected in May 2011 at permitting. The site is accessed by a <br /> dirt road extending south along the permit line and then west to the Phase 1 are of the pit. At the time of this <br /> inspection, the site was dry and no evidence of standing water was observed. A crusher and conveyor were <br /> located in the southern part of the pit, as well as stockpiles of material and overburden.The entrance road <br /> was marked with the permit sign. The permit area is 155.2 acres with 15 acres allowed to be disturbed at any <br /> one time. According to the Annual Report dated August 2016, approximately 12 acres of the allotted 15 acres <br /> of affected area have been disturbed. <br /> The affected area is located in the north central portion of the permit area and includes two phases of mining. <br /> Phase 3 has not yet begun, but will be located west of Phase 2. Topsoil was observed around the perimeter of <br /> both Phase 1 and phase 2 pits. The topsoil stockpiles were vegetated and stable. The pit slopes are at a 3:1 <br /> grade or flatter. Portions of Phase 1 and Phase 2,totaling about 1.5 acres, have been reclaimed to the point of <br /> reseeding. Some grass growth is occurring as well as some weed growth. The Operator should implement <br /> weed control measures and reseed these areas to keep the areas moving toward_ reclamation and eventual <br /> release. <br /> The site is generally stable and in good condition. <br /> RECOMMENDATIONS: <br /> 1. The Operator should implement weed management on the site to reduce the weed growth. This is <br /> not being cited as a problem at this point, but without weed control measures, may be in the <br /> future. <br /> 2. Reseeding may be needed to establish beneficial growth in the areas that are working toward <br /> reclamation. <br /> 3. Bond evaluation should be done at the time of the next DRMS inspection. <br /> Page 2 of 6 <br />
