PERMIT#: M-2008-013 <br /> INSPECTOR'S INITIALS:TCW <br /> INSPECTION DATE: October 19,2016 <br /> OBSERVATIONS <br /> This was a routine monitoring inspection conducted by TC Wait of DRMS; Mr. Rudy Torres was present <br /> representing the operator. The site is located about 1.5 miles west of Hasty off of County Road JJ, north of the <br /> John Martin Reservoir. The site was last inspected in January 2012. The permit area is 9.9 acres. Based on <br /> aerial imagery(GoogleEarth, 3/19/2016), the disturbed area is approximately 9.2 acres. <br /> At the time of this inspection,the site was dry and mining operations were occurring along the southern side <br /> of the site. The entrance road was marked with the permit sign and the permit boundaries were marked with <br /> fence line and corner signs. <br /> No processing equipment was onsite. Several stockpiles of material were observed in the pit floor.Topsoil has <br /> been salvaged and piled at the east side of the pit. Mr.Torres indicated that portions of the north side of the <br /> pit had been regraded to approximately 5:1 slope and are working to revegetate this area (estimated to be 1.5 <br /> acres). The vegetation has not yet established, and the area did appear stable. <br /> Some rilling erosion was noted on the stockpiles located in the pit. No evidence of standing water was <br /> observed in the pit. Minor weed growth was observed in the pit area. <br /> The recent (April 2016) annual report mine map for this site is not accurate and does not reflect the actual <br /> location of the mine operation as observed during the inspection. Based on the annual report map, the pit is <br /> located directly south of County Road JJ; however, when paced in the field, the actual distance is <br /> approximately 1500 feet south of the county road. DRMS will need accurate GPS-derived locations of the <br /> corner markers for this site to update the map. <br /> The site is generally stable and in acceptable condition. <br /> RECOMMENDATIONS: <br /> 1. The Operator should begin site grading and revegetation toward reclamation in areas where mining <br /> has finished. <br /> 2. The Operator needs to provide DRMS with accurate GPS-derived locations for the permit corners to <br /> update the maps on file. This has been cited as a problem and requires corrective action. <br /> 3. Bond evaluation should be done at the time of the next DRMS inspection. <br /> Page 2 of 8 <br />
