Oldcastle SW <br /> Group,Inc.dba United Companies of Mesa County is now the permitted operator of the Mamm Creek Sand and Gravel Pit,and as such, <br /> is responsible for all provisions in M-2000-113,as well as those specified in the Rules and Regulations. Lafarge West,Inc.is relieved of <br /> all responsibilities concerning this operation. <br /> Stipulation No.2 <br /> Oldcastle SW Group, Inc. dba United Companies of Mesa County to submit all required properly executed structure damage waiver <br /> agreements within 180 days of this approval of Succession of Operator. If waivers cannot be obtained within 180 days,the operator shall <br /> submit a geotechnical evaluation including engineering stability analysis as requiredper CRS 34-32.5-116(4)(i)and Rule 6.5. <br /> On April 1, 2014 the Division received the properly executed Structure Damage Waiver Agreement from the landowner James G. <br /> Snyder.
