The following inspection topics were identified as havinp, Problems or Possible Violations. OPERATORS
SHOULD READ THE FOLLOWING PAGES CAREFULLY IN ORDER TO ASSURE COMPLIANCE
WITH THE TERMS OF THE PERMIT AND APPLICABLE RULES AND REGULATIONS. If a
Possible Violation is indicated, you will be notified under separate cover as to when the Mined Land
Reclamation Board will consider possible enforcement action.
INSPECTION TOPIC:Administrative
PROBLEM/POSSIBLE VIOLATION: Problem:The annual report submitted by the Operator on September 7, 2016
does not include a map that accurately depicts the permit boundary, current affected area boundary, and
location of acreages per Colorado Land Reclamation act for the Extraction of Construction Materials (CRS 34-
32.5-116) and Rule 6.3.5.
CORRECTIVE ACTIONS:The Operator shall submit to the Division a corrected annual report map depicting the
accurate permit boundary(defined with GPS coordinates), current affected area boundary, and location of
acreages for this operation.
