PERMIT#: M-2007-050 <br /> INSPECTOR'S INITIALS:TCW <br /> INSPECTION DATE: October 18,2016 <br /> OBSERVATIONS <br /> This was a routine monitoring inspection conducted by TC Wait of DRMS. The Division left messages for Mr. <br /> Yocam about the inspection, however a representative of the Operator was not present at the time of the site <br /> visit. The site is located about 8 miles north of US 160 on CR 223 between Kim and Pritchett. The site was last <br /> inspected in October 2012. At the time of this inspection,the site was dry and no active mining operations <br /> were occurring. The entrance road was marked with the permit sign and the permit boundaries were marked <br /> with t-posts. The haul road was vegetated and did not appear to have been used recently. <br /> Topsoil is stockpiled on the north and south sides of the pit. Stockpiles of material are present on the west <br /> side of the excavation. No reclamation work appears to have been done at this point. Based on aerial <br /> photography (GoogleEarth 3/19/2016), approximately 2.4 acres has been disturbed. <br /> No evidence of standing water was observed in the pit. Weeds were not observed in the pit area. Minor trash <br /> (buckets, palates, etc.) were observed around the work area. <br /> The site is generally stable and in good condition. <br /> RECOMMENDATIONS: <br /> 1. If the site is not currently active, a Temporary Cessation application should be considered. <br /> 2. Bond evaluation should be done at the time of the next DRMS inspection. <br /> Page 2 of 6 <br />
