PART ii <br />Page No, is <br />Permit NO COO- 500000 <br />The permittee, or the duly authorized representative shall make and sign the following certification on all such documents: <br />"I certify under penalty of law that this document and all attachments were prepared tinder my direction or supervision in <br />accordance with a system designed to assure that qualified personnel properly gather and evaluate the information submitted. <br />Based on my inquiry of the person or persons who manage the system, or those persons directly responsible for gathering the <br />information, the information submitted is to the best of my knowledge and belie$ true, accurate and complete. I am aware that <br />there are significant penalties for submitting false information, including the possibility of fine and imprisonment for knowing <br />violations." <br />PART II <br />A. The alteration or addition could significantly change the nature or increase the quantity of pollutants discharged, or, <br />b. The alteration or addition results in a significant change in the penmittee's sludge use or disposal practices, and such alteration, <br />addition, or change may justify the application of permit conditions that are different from or absent in the existing permit, <br />including notification of additional use or disposal sites not reported pursuant to an approved land application plan. <br />The permittee shall give advance notice to the Division of any planned changes in the permitted facility or activity which may result <br />in noncompliance with permit requirements. <br />Whenever notification of any planned physical alterations or additions to the permitted iitcility is required pursuant to this section„ <br />the puttee shall furnish the Division such plans and specifications which the Division deems reasonably necessary to evaluate the <br />effect on the discharge, the strearn, or ground water. If the Division finds that such new or altered discharge might be inconsistent <br />with the conditions of the permit, the Division shall require a new or revised permit application and shall follow the procedures <br />specified in Sections 61.5 through 61.6, and 61.15 of the Colorado Discharge Permit System Regulations. <br />
