The permittee, or the duly authorized representative shall make and sign the following certification on all such documents:
"I certify under penalty of law that this document and all attachments were prepared tinder my direction or supervision in
accordance with a system designed to assure that qualified personnel properly gather and evaluate the information submitted.
Based on my inquiry of the person or persons who manage the system, or those persons directly responsible for gathering the
information, the information submitted is to the best of my knowledge and belie$ true, accurate and complete. I am aware that
there are significant penalties for submitting false information, including the possibility of fine and imprisonment for knowing
violations."

Notice is required only when:
a. The alteration or addition could significantly change the nature or increase the quantity of pollutants discharged, or,
b. The alteration or addition results in a significant change in the penmittee's sludge use or disposal practices, and such alteration,
addition, or change may justify the application of permit conditions that are different from or absent in the existing permit,
including notification of additional use or disposal sites not reported pursuant to an approved land application plan.
The permittee shall give advance notice to the Division of any planned changes in the permitted facility or activity which may result
in noncompliance with permit requirements.
Whenever notification of any planned physical alterations or additions to the permitted iitcility is required pursuant to this section„
the puttee shall furnish the Division such plans and specifications which the Division deems reasonably necessary to evaluate the
effect on the discharge, the strearn, or ground water. If the Division finds that such new or altered discharge might be inconsistent
with the conditions of the permit, the Division shall require a new or revised permit application and shall follow the procedures
specified in Sections 61.5 through 61.6, and 61.15 of the Colorado Discharge Permit System Regulations.
