As a result of our review, we have
identified a deficiency in the signed Agreement that will need to be resolved before an
ePermitting account can be created for your company.
1. The final paragraph of the agreement form sent to you includes the following language:
This signature on this Agreement must be notarized, and if the signature is that of a
company, corporation, partnership, or governmental agency representative, must be
accompanied by a notarized affidavit of signing authority of the individual for that
entity.
We have since revised the language of the Opt-In agreement for clarification:
This signature on this Agreement must be notarized, and if the signature is that of a
company, corporation, partnership, or governmental agency representative, must be
accompanied by:
a) the attached notarized affidavit of signing authority of the individual for that
entity, or
b) a resolution of the decision-making body of your company that authorizes an
individual to sign the ePermitting Agreement on the company's behalf.
'pp•c0 O
1313 Sherman Street, Room 215, Denver, CO 80203 P 303.866.3567 F 303.832.8106 http://mining.state.co.us
John W.
