Copies have also been provided to the Office of Surface Mining Reclamation and <br />Enforcment (OSMRE) at 1999 Broadway, Suite 3320, Denver, CO, and the Bureau of Land Management <br />(BLM) Little Snake Field Office at 455 Emerson Street, Craig, CO. <br />2.03.3(9) Certifcation by Responsible Offical <br />The information contained in this permit application is true and correct to the best of current knowledge, <br />information and belief. The responsible offical has certified this permit application in accordance in the <br />rules in the cover letter application. <br />2.03.4 Identification of Interests <br />Colowyo Coal Company L.P. is a limited partnership between two wholly owned subsidiaries of Western <br />Fuels — Colorado LLC. It's General Partner and Manager is Axial Basin Coal Company, which hold a <br />20% partnership interest in the Colowyo Mine. The remaining 80% partnership interest is held by Taylor <br />Creek Holding Company. Western Fuels — Colorado is owned by two entities, Western Fuels <br />Association, Inc. (1%) and Tri-State Generation and Transmission Association (99%). <br />Western Fuels — Colorado, a Limited Liabiliyt Company (WFC), is the permit applicant and mine <br />operator for the Colowyo Mine (Colowyo). The general office is located at 5731 State Highway 13, <br />Meeker, Colorado 81641; telephone number (970) 824-1200. Colowyo's Employer Identification <br />Number is 87-0522903. Colowyo's Mine Safety and Health Adminstration (MSHA) identification <br />number is 05-02962 and it was issued on October 1, 1976. <br />The duly Authorized Representative for WFC is Barbara Walz, Chief Compliance Officer for Tri-State <br />Generation and Transmission Association, Inc.
