Dear
Your home, shop or other man made structure is located within the pre-blast survey area of the Hitch Rack Ranch
Quarry. The site is located in along Highway 115 in Southern El Paso County.
We will be taking precautions at our blasting operations,and intend to prevent damage to your structure by following
restrictions established by the mining permit and state and federal agencies. We are offering,in good faith,a pre-blast
survey to every resident and owner of a structure within the pre-blast survey radius of one-half mile from the proposed
disturbance boundaries for this mine permit. The pre-blast survey will record and document the physical condition of
your structure prior to any blasting. The pre-blast surveyor will look at each room in your home as well as each
outside wall and document the current condition using one or more of the following: a camera, sketches, drawings,
narrative descriptions or a combination of methods. If your water supply is a well or spring,a water sample will be
taken and an analysis of the untreated water will be included with the survey.The surveyor will also measure the water
level in the well and the well production rate if the well can be accessed.
Pre-blast survey request and waiver forms are included with this letter. If you would like to have a pre-blast survey
performed at no cost to you,please complete the"request'section of the form. If you do not want a pre-blast survey,
please fill out the"waiver"section of the form. A stamped addressed envelope is included for your convenience. If
we do not receive the form back from you before ,we will assume that you have declined a pre-blast survey.
A copy of the completed pre-blast survey will be sent to the Colorado Department of Reclamation,Mining,and Safety
(DRMS) for their records. Transit Mix Concrete Co. will keep a copy, and the structure owner will be provided a
copy. If you disagree with any results determined by the survey,you should send a written explanation detailing the
points of disagreement to DRMS and Transit Mix Concrete Co. Your letter explaining the disagreement will be made
a part of the pre-blast survey file.
If you expand or renovate your structure,another survey of the renovations/additions will be performed at your request.

of why you want an up-dated pre-blast survey.
