Before the Survey—Transit Mix Concrete Co. will send notifications by certified mail to <br /> neighbors at least 30 days prior to any blasting being conducted on site. The survey will be <br /> complimentary to the neighbor and will be conducted by an independent third parry consulting <br /> firm that specializes in pre-blast surveys and blast monitoring. Included in the notification will <br /> be a description of the process that will take place on the day of the inspection, what the <br /> inspector will be looking for, and how the inspection will be documented. Also included with the <br /> notification will be a survey response form that each neighbor will complete and send back to <br /> Transit Mix Concrete Co. This form will be used as consent to complete the survey or as <br /> statement by the landowner declining the survey. A lack of response will be deemed to indicate <br /> their declining participation in the survey. Once the form is returned and a survey requested, a <br /> representative from the independent blasting consulting firm will contact the neighbor to <br /> schedule a date and time to complete the survey. Transit Mix Concrete Co. recommends that the <br /> landowner collect any documents relevant to the structures or wells that will be evaluated; these <br /> may include date and details on the structure construction and well drilling and completion <br /> reports. <br /> A copy of the Pre-Blast Survey notification is included in Appendix A. <br /> During the Survey—On the day of the survey the representative from the blasting consulting <br /> firm will arrive on the property, introduce themselves, and conduct the inspection. Any available <br /> documents will be reviewed at the start of the survey. The consulting representative will have a <br /> still photo camera and a video camera. The representative doing the inspection will photograph <br /> and video document, with voice comments,the exterior and interior of all structures located on <br /> the property. They will document the existing condition of all walls, doors, ceilings, windows <br /> and floors. The survey will include all levels of the structure including the basement and <br /> foundation. Areas that cannot be accessed during the inspection will be noted. The current <br /> conditions of the structure will also be recorded in the form of notes and sketches on a standard <br /> inspection form used by the consulting company. <br /> In addition to the structure inspection, the representative will also assess the condition of any <br /> water wells within the established distance. If the well is accessible,this will include measuring <br /> the water level in the well and conducting flow rate testing. The flow rate testing will involve <br /> pumping the well for a minimum of 15 minutes to determine the well production rate and water <br /> level. Water level measurements will not be performed if the well is not accessible. Flow rate <br /> testing will be performed if there is a spigot between the well and cistern/pressure tank. The <br /> surveyor will also collect a water quality sample. If the well production can be tested, then the <br /> water quality sample will be collected after the well production test from the pumped flows. <br /> Transit Mix Concrete Co. may work with the landowner to have the surveyor open the well for <br /> testing, and this work may include removing the pump from the well. Regardless of well <br /> accessibility, a water quality sample will be collected from the spigot closest to the well and/or <br /> cistern. A minimum of 1 gallon of water will be discharged prior to collecting the sample. The <br /> water quality monitoring parameter list is based on Domestic Water Supply numeric standards <br /> established by the Colorado Department of Public Health and Environment and public <br /> comments. This list is included as Table 1. The water sample will be sent to an independent <br /> Colorado state certified lab that will test for water quality. <br />
