I, Jared Ebert of the Colorado Division of Reclamation, Mining and <br />Safety (Division) conducted the inspection. Mr. Brian Watterson with PSCM accompanied me during the <br />inspection. The weather was clear and warm during the inspection. <br /> <br />HYDROLOGIC BALANCE - Rule 4.05 <br />Drainage Control 4.05.1, 4.05.2, 4.05.3; Siltation Structures 4.05.5, 4.05.6; Discharge Structures 4.05.7, 4.05.10; <br />Diversions 4.05.4; Effluent Limits 4.05.2; Gr ound Water Monitoring 4.05.13; Surface Water Monitoring 4.05.13; <br />Drainage – Acid and Toxic Materials 4.05.8; Impoundments 4.05.6, 4.05.9; Stream Buffer Zones 4.05.18: <br />Pond 004 was observed. PSCM has removed the original corrugated metal spillway pipe that was <br />decommissioned in 2005 that began to leak significantly earlier this summer. The culvert spillway was removed <br />and the embankment was re-established. According to Mr. Watterson, PSCM had hired an engineering firm to <br />conduct compaction tests as they reestablished the embankment. This firm is preparing a final report. The face of <br />the slope was measured to be 40% where the repair work was completed. Mr. Watterson indicated the contractor <br />had seeded the embankment but PSCM may re-seed the embankment repair area this fall. The top widt h of the <br />embankment was 14 feet where the repair work was completed. The embankment appears t o have been re- <br />established in accordance with the original as -built depicted on permit map 2.05.3-E2.C. The trash rack on the <br />approved primary spillway is damaged and needs to be repaired.
