Attached photos show the exposed water before and after the backfill. <br /> The well at the Wyman Gravel Pit(Permit# M-1983-037)is a water monitering well installed to a depth of 16'. <br /> This well is part of a CDPHE(Colorado Department of Health and Environment)closure plan for a remediated <br /> wastewater treatment plant(Pagoda Pure Water Recycling Facility)for the owner Louis M Wyman, This is one <br /> well of 4 that was installed during the closure project in 2009. This well is designated MW-4A and is one of 4 <br /> that is required to be tested twice a year for another 20 years by the CDPHE, The designation from the CDPHE <br /> for this well is MW-4A for Project No.4246.001 under the Routt County Department of Environmental Health <br /> Permit No. 00-331. A Site map showing the well location relative to the remediated project and a letter to <br /> Amend Certificate of Designation is also attached. <br /> If any additional information is required, please let me know. <br /> Thank You <br /> David L.Wyman <br />
