Response to Second Adequacy Review and Technical Revision
Dear Eric:
Aggregate Industries(AI) submits this letter together with the enclosures referenced herein as its response
to the Division's June 26, 2016 second adequacy review items, including a technical revision to AI's
application for an amendment to Permit No. M-2004-031, Hazeltine Mine.
6.4.3 EXHIBIT C -Pre-Mining and Minin$Plan Map(s)of Affected Lands
In response to Item 12 of the Division's second adequacy review, Pre-Mining and Mining Plan Maps
approved through the original application have not changed except for ownership of record of some of the
adjoining parcels of land. A list of the owners of record of surface and mineral rights and owners of
record of land within 200 feet of the affected area is therefore enclosed as an amendment to the list of
Property Owners within 200 feet as set forth in the currently approved pre-mining map. The parcel
numbers on the enclosed amended list correspond with the parcel numbers on the currently approved pre-
mining map. The enclosed list was updated at the time of the filing of this amendment application and
notice was mailed to the owners of record at that time. Copies of return receipts of such mailing were
included with AI's June 21,2016 response to the Division's preliminary adequacy review.
6.4.5 EXHIBIT E -Reclamation Plan
In response to Division adequacy items 1 and 8, and as the technical revision element regarding reservoir
outlet pipes required by the Division under General Questions and Comments item 11, enclosed is
Tetra Tech Memo dated August 24,2016,Orr Property Drain and Reservoir Outlet Pipes and associated
drawings.
In response to adequacy item 9, Al commits to submitting to the Division within 60 days of
completion of the permanent groundwater mounding mitigation plan, a certification created by a
qualified engineer certifying that the permanent plan has been constructed to the required; design,
compaction, and specifications approved through AM-01.
6.4.6 EXHIBIT F-Reclamation Plan Map
In response to Item 10 of the Division's second adequacy review, enclosed is the revised drawing
prepared by Tetra Tech.
