Everist, Inc. <br /> Dear Division: <br /> L.G. Everist, Inc., respectfully submits the Annual Report for the Firestone Gravel Resource, DRMS <br /> Permit# M-1996-052, along with a check for $791.00 to cover the Annual Fee. The Annual Report <br /> includes a Mining Map for a visual representation of the affected areas. <br /> The affected area has not changed much since the last annual report. We have continued mining in <br /> the western half of the site, and constructed a large settling pond in the southwest corner of the site. <br /> The settling pond's perimeter slopes were sloped with semi-compacted materials. We have seeded a <br /> few areas on the west side of the site to encourage grass growth and control weeds. We also added a <br /> small boneyard along the south. This summer we are completing the construction of a concrete batch <br /> plant, which is located along the west side of the site in the plant area shown in our approved permit <br /> materials, and it is situated between the main entrance and the mine processing plant. <br /> Also attached as required is a Groundwater Monitoring Report for the site. The report includes recent <br /> data and a copy of the Monitoring Well map. Analysis of the groundwater monitoring data showed that <br /> all exterior wells have remained in compliance. Two wells which are interior to our operation (MW02, <br /> MW14) have been outside the baseline ranges a few times, but because the wells are interior they will <br /> not adversely affect any neighbors.
