PERMIT #: M-2013-026 <br />INSPECTOR’S INITIALS: DMC <br />INSPECTION DATE: August 17, 2016 <br /> <br /> <br />Page 2 of 4 <br />OBSERVATIONS <br />This inspection was conducted as part of the Division of Reclamation, Mining and Safety’s (Division) normal <br />monitoring program. The Operator was not present during this inspection. Photographs are included with this <br />report in order to illustrate some of the conditions observed. <br />The Hopemore Shaft is located approximately 2.5 miles east of Leadville and accessed from CR1. The site is a <br />110 operation that includes a total of 9.9 permitted acres. The Division currently holds an adequate financial <br />warranty amount of $32,500.00 for this site. <br />Adequate mine identification signage was noted at the entrance to the site. <br />The affected area was marked by posts that were clearly visible and adequate to delineate the affected area <br />boundary. <br />The area around the Hunter Shaft was orderly. The buildings were secured with padlocked doors. <br />The area around the Hopermore Shaft was orderly. The buildings were secured with padlocked doors. A small <br />pile of wasterock was located below the ore chute. There was no ore stored on the ore pad. Evidence of runoff <br />leaving the ore pad was observed.
