PERMIT#: P-2015-021 <br /> INSPECTOR'S INITIALS: MAC <br /> INSPECTION DATE: July 21,2016 <br /> OBSERVATIONS <br /> The inspection was conducted by Michael Cunningham of the Division of Reclamation, Mining and Safety <br /> (Division) and Amy Titterington with the U.S. Forest Service. The Operator was not present for the inspection. <br /> The LGPD2-2 prospect site is located in Park County, approximately 1.2 miles north of Lake George in the Pike- <br /> San Isabel National Forest. The commodities of interest are quartz, amazonite and flourite. <br /> The Prospector is approved to move 284 cubic yards of material which will be excavated from five trenches <br /> (16' x 16' x 6'). The Division observed several excavations which were approximately one cubic yard in size. In <br /> addition, there was a larger trench which was approximately 3' wide, 4' deep and 20' in length. No equipment <br /> was observed on site and it appeared the trench had been excavated with hand tools. <br /> Following the inspection, the Division had a telephone conversation with Mr. John Crowley who stated that <br /> the affected area observed during the inspection was created by claim jumpers. Mr. Crowley informed the <br /> Division that his prospecting project had concluded and that four of the five approved pits had been excavated <br /> and backfilled. Mr. Crowley is preparing to seed the reclaimed trenches within the next several weeks. In <br /> addition, Mr. Crowley will send the Division a letter documenting the disturbance which is in the vicinity of the <br /> LGPD2-2 prospect site. Upon receipt of the letter from Mr. Crowley, the Division may schedule a follow-up <br /> inspection to document the reclaimed areas. <br /> The Division holds a financial warranty in the amount of$2,625.00, which remains adequate to complete <br /> reclamation. <br /> This concluded the inspection. <br /> Page 2 of 3 <br />
