The Applicant/Operator name must be listed exactly as it is registered with the Colorado Secretary of State: Arkins Park Stone Corporation. Please revise the Applicant/Operator name in both places on Page 8 of the Application Form. 5. Pursuant to Rule 1.6.2(1)(b), the Applicant is to provide a signed affidavit that the required public notice has been posted at the location of the mine site. Please complete and return the enclosed affidavit certifying the public notice has been posted at the mine site. 6.4.7 Exhibit G—Water Information 6. The Applicant did not include any information for Exhibit G. Pursuant to Rule 1.10(1),the Applicant will not be required to submit any information which duplicates applicable previous submittals. However, the Applicant shall clearly describe where in the original application the information not included in the Amendment Application, but necessary to render the Amendment technically adequate, may be found. Please provide proof that notice of the Amendment Application was sent to the local Board of County Commissioners and the Soil Conservation District. Proof of notice shall be in the form of a return receipt of a Certified mailing or a date-stamped copy of the notice acknowledging receipt by the appropriate Board. 6.4.18 Exhibit R—Proof of Filing with County Clerk and Recorder 8. The Applicant did not submit proof the Amendment Application was filed with the County Clerk and Recorder for public review. Proof of filing shall be an affidavit or receipt indicating the date on which the Amendment Application was placed with the County Clerk and Recorder for public review. 6.4.19 Exhibit S—Permanent Man-made Structures 9. The Applicant did not include any information for Exhibit S. Pursuant to Rule 1.10(1), the Applicant will not be required to submit any information which duplicates applicable previous submittals. However, the Applicant shall clearly describe where in the original application the information not included in the Amendment Application,but necessary to render the Amendment technically adequate, may be found.
