PERMIT#: M-2001-017 <br /> INSPECTOR'S INITIALS: ECS <br /> INSPECTION DATE:August 3,2016 <br /> OBSERVATIONS <br /> This was a pre-operation site inspection conducted by DRMS at the Carbon Valley Resource site. LG Everist <br /> submitted an application for AM01 that was called compete on June 24, 2016. The inspection was conducted <br /> by Eric Scott of CDRMS. The DRMS representative met with Lynn Mayer-Shults of L.G. Everist at the site at <br /> approximately 1130. The site entrance is located off the east side of Hwy 7 approximately 1 mile south of <br /> where it intersects Highway 119 (also known as Firestone Boulevard). The site identification sign and AM01 <br /> notice posting were both present at the site entrance (Photo 1). <br /> DRMS toured the site with the L.G. Everist representatives. Current activity in the mine consists of mining in <br /> Area B, cleaning out the pit bottom, installing a compacted clay liner, and re-sloping the pit walls to the <br /> approved 3:1 final reclamation slope. Area A is in the process of being backfilled to approximate original grade <br /> with wash fines and imported inert fill material. The office and processing areas of the site are as depicted on <br /> the mining plan (Photos 2-4) <br /> AM01 was submitted to add an approximately 56 acre parcel to the west side of site between the existing <br /> permit and Weld County Road 13. The parcel is currently an irrigated corn field with some cattle grazing at the <br /> south end of the area. A residential property is located immediately NW of the proposed permit area. The <br /> parcel and features present (buildings, utilities, etc.) appeared to match well with the amendment application <br /> submitted to DRMS (Photos 5-6). <br /> The southern boundary of the permit area will continue to be the centerline of the Last Chance irrigation <br /> ditch. Fences mark the proposed permit boundary of the new parcel along the west and a portion of the north <br /> side, however, the permit boundary will need to be established across the remaining unmarked NW corner of <br /> the site and along the south border with the irrigation ditch. Please submit proof to DRMS that this has been <br /> completed prior to the AM01 decision date. Required setbacks, such as from the utility easement along the <br /> west side of the site, should also be clearly marked during mining to prevent accidental encroachment. <br /> During the site tour, the mine appeared to be in compliance with the approved mining and reclamation plan. <br /> The only problem noted during the inspection was the presence of noxious weeds at the site (cheatgrass, <br /> bindweed, thistle, etc). These noxious plants need to be eliminated before they become established and <br /> widespread. In areas still to be reclaimed, this could easily be done using a dozer when the area is re- <br /> contoured or filled during reclamation. Other areas will require weed control measures such as spraying or <br /> mowing. The presence of noxious weeds has been cited as a problem in prior reports as well. <br /> This concluded the inspection and DRMS left the site at approximately 1330. <br /> Inspection Contact Address <br /> Lynn Mayer Shults <br /> L.
