As a result of our review, we have identified <br />deficiencies in the signed Agreement that will need to be resolved before an ePermitting account can be <br />created for your company. <br /> <br />1. The signature page of the Agreement was incomplete; see the annotated copy of Diamond B <br />Enterprises, LLC’s executed agreement (attached) for details on the information that is missing or <br />incorrect. Please submit a newly notarized replacement signature page for the Agreement that <br />provides all of the required information. <br /> <br />2. The final paragraph of the agreement form sent to you includes the following language: <br /> <br />This signature on this Agreement must be notarized, and if the signature is that of a company, <br />corporation, partnership, or governmental agency representative, must be accompanied by a <br />notarized affidavit of signing authority of the individual for that entity. <br /> <br />We have since revised the language of the Opt-In agreement for clarification: <br /> <br />This signature on this Agreement must be notarized, and if the signature is that of a <br />company, corporation, partnership, or governmental agency representative, must be <br />accompanied by: <br /> <br />a) the attached notarized affidavit of signing authority of the individual for that entity, or <br /> <br />b) a resolution of the decision-making body of your company that authorizes an individual <br />to sign the ePermitting Agreement on the company’s behalf.
