2016 Annual Report and Fee

Dear Mr. Clement:

On April 29, 2016, the Division sent you an electronic mail message informing you that the
$323.00 fee you attempted to submit on March 2, 2016 with your annual report for Permit No.
M1997084 was not collected by the Division. At the time of your March 2, 2016 payment
attempt, we were having problems with the payment service and credit card payments were not
being finalized. In the April 29, 2016 electronic mail message, we asked you to resubmit your
payment.

To date, we have not received the $323.00 annual fee payment. If the fee is not received by June
8, 2016, you will be in violation of C.R.S. 34-32.5-116(3)(a) and (b) and the terms and conditions
of Permit No. M1997084.

As you were previously notified, we were able to retain the information you submitted in your
report, as well as any attachments (such as your map) that you submitted, so you will not need
to reenter any of that information. You should have received an email from our system with a
link to your draft report. Please open the email, click on the saved draft link, proceed to the "Pay
Now" button on the report, and resubmit your payment.
