As a result of our review, we have identified <br />a deficiency in the signed Agreement that will need to be resolved before an ePermitting account <br />can be created for your company. <br /> <br />1. The final paragraph of the agreement form sent to you includes the following language: <br /> <br />This signature on this Agreement must be notarized, and if the signature is that of a <br />company, corporation, partnership, or governmental agency representative, must be <br />accompanied by a notarized affidavit of signing authority of the individual for that <br />entity. <br /> <br />We have since revised the language of the Opt-In agreement for clarification: <br /> <br />This signature on this Agreement must be notarized, and if the signature is that of a <br />company, corporation, partnership, or governmental agency representative, must be <br />accompanied by: <br /> <br />a) the attached notarized affidavit of signing authority of the individual for that <br />entity, or <br /> <br />b) a resolution of the decision-making body of your company that authorizes an <br />individual to sign the ePermitting Agreement on the company’s behalf.
