PERMIT#: P-2016-005 <br /> INSPECTOR'S INITIALS: MAC <br /> INSPECTION DATE:July 21,2016 <br /> OBSERVATIONS <br /> The inspection was conducted by Michael Cunningham of the Division of Reclamation, Mining and Safety <br /> (Division) and Amy Titterington of the U.S. Forest Service (USFS). The Operator was not present for the <br /> inspection. The Puma City site is located approximately 9 miles northwest of Lake George in Park County <br /> Colorado. The Puma City prospect site is on an unpatented claim in the Pike National Forest. <br /> The Division received a request for a full financial warranty release on July 11, 2016. As required by Rule <br /> 4.16.1(3), the Division is required to inspect the site within 30 days of receiving a release request in order to <br /> determine if the Operator reclaimed the site in accordance with the approved Reclamation Plan and all other <br /> applicable requirements. The prospecting activities at the Puma City NOI included excavating 2 cubic yards of <br /> material to access an inclined shaft for the purpose of collecting samples. All prospecting activities were <br /> carried out with hand tools and the excavation occurred on an existing waste rock pile. <br /> The Prospector replaced all excavated material which was removed to access the inclined shaft. Minimal <br /> vegetation was disturbed during prospecting and the Prospector salvaged and replaced grasses which were <br /> growing in the excavated area. The pre-prospecting and post-prospecting photographs provided by the <br /> Prospector show the site was reclaimed to the same condition it was in before prospecting occurred. <br /> Based on the observations made during this inspection, the Prospector has successfully completed <br /> reclamation of the Puma City NOL Notice of the Division's decision regarding the release request will be sent <br /> under separate cover. <br /> Page 2 of 3 <br />
