OTTEN <br /> Central City Consolidated Mining Co. <br /> 11438 County Road 19 <br /> Fort Lupton, CO. 80621 JUL 2 Q 2016 <br /> (970)785-2538 <br /> DIVISION OF REDLAA40 <br /> MINING&SAFETY <br /> � <br /> July 18,2016 <br /> Mr. Michael Cunningham <br /> Division of Reclamation,Mining and Safety <br /> 1313 Sherman St.,Room 215 <br /> Denver, CO 80203 <br /> RE: M1990-041,Inspection 4/26/16,Corrective actions;Request for extension <br /> Dear Mr. Cunningham, <br /> In regards to the Problem noted on your Inspection Report of 4/26/16 (liquids <br /> overtopping secondary containment,etc.),Central City Consolidated Mining Co. hereby <br /> requests additional time for the completion of corrective actions. <br /> We have contracted Black Fox Mining LLC to address the problem as stated by the <br /> Division and to perform remediation. All liquids and soils to be removed shall be done <br /> according to the Divisions' stated corrective actions. Any materials to be retained on the <br /> site shall be located within the Water Treatment Building. The Water Treatment <br /> Building has an internal sump of dimensions: 12.5 feet long x 2.5 feet wide x 5 feet deep, <br /> able to contain approximately 1,100 gallons of liquid. Currently,this sump is one-half <br /> full of liquids(water),allowing for the emergency containment of an additional 550 <br /> gallons of liquid, should a spill occur. No allowance has been included for the volume of <br /> the trough drains, which would also contain liquids within the building. Additional spill <br /> containment exists within the building(spill containment pallet,etc). <br /> Central City Consolidated Mining Co. has attached the contract with Black Fox Mining <br /> LLC and the plans for the corrective actions as evidence of our commitment to resolving <br /> this issue. <br /> At this time, Central City Consolidated Mining Co. request a 60 day extension to <br /> complete the tasks, so that our contractors and service providers may fully execute the <br /> measures necessary to mitigate the problem and prevent reoccurrence. <br /> Mr. Michael Cunningham 7/18/2016 <br /> Division of Reclamation,Mining and Safety Page 1 of 2 <br /> M1990-041: Corrective Actions—Extension Request <br />
